The RideAbility Student Application should be filled out to provide contact and scheduling information to RideAbility. The second purpose of this form is to let RideAbility know what special needs the client has. These are important to take into consideration while evaluating safety plans and setting goals. This form is also the best place to document the desires the client and families have for their involvement at RideAbility.
For returning clients, there is plenty of space to write-in comments and feedback on the back page!
