Participation form

The RideAbility Participation Form includes all industry standard required signatures (such as liability releases, photo releases, affirmation of program orientation, and confirmation of notification of the Minnesota Equine Liability law).     This form must be signed and submitted to RideAbility before a student can be allowed to enter the arena at the barn or be near the horses.

This form should be filled out, signed and submitted to RideAbility annually to ensure current contact information and provide recent liability releases.

The hardcopy of this form will be kept in the student’s individual file.
